NRLP offers centralized and consistent communications, protocols, training, policies, and procedures across all operations in three field offices located in Lynn, Lowell, and Danvers.

NRLP is creating an elite corps of LTSS Coordinators with expertise in multiple areas ranging from outreach and engagement; conflict-free care management; person-driven care planning; wellness coaching; working with high-risk populations; data gathering and documentation; and care transitions.

Capacities for Comprehensive Data Exchange

NRLP will facilitate data exchange through our Thrasys/Syntranet technology platform, providing state-of-the-art EHR, care management software, data management, and communications capabilities.

 

Robust Predictive Analytics

NRLP will have the capacity to integrate demographics, social determinants of health, and clinical data to support timelier and more precisely focused interventions that can effectively promote stronger health outcomes and reduce related costs.
